Please put down several options for your Fall availability, ranked from most preferable to least preferable. All appointment days and times are subject to clinician availability. We are open Monday through Friday, 8am-6:30pm.

Preferred Clinician

If you/your child receives more than one service at NETC please list the clinician for each service.

Do you need a new Release of Information Consent Form to be shared?

If you'd like your/your child's clinician to be able to share information with outside therapists, such as school therapists or other private practice therapists, this is required.

Are you open to a new clinician if your current clinician isn't available at your preferred time?
